- Do you have or have you had in the last 2 weeks, a fever, cough, sore throat, loss of smell/taste or any other flu like symptoms?
- Have you been in contact with anyone who has a confirmed diagnosis of COVID-19 in the past 14 days?

We ask that all patients arrive no more than 5 minutes before your session. If you arrive early, you may wish to wait in your car or the lobby downstairs to reduce the number of people in the waiting area. Please also ensure that you wear a mask while attending your appointment. If you do not have one, a mask will be provided.
Appointments will be spread out and schedules staggered in order to allow time for proper cleaning and disinfection. All surfaces will be appropriately disinfected after patient visits according to Public Health guidelines and high traffic contact surfaces will be disinfected at regular intervals throughout the day. As well, our staff will be wearing protective gear according to the guidelines provided by Public Health.
For a more contactless appointment, we will be accepting email money transfers as a method of payment for your exam as well as for contacts and glasses. Simply add Dr. Jiwani as a contact with the email address email@example.com.
